Coordination of Supports

NDIS Levels of Support Coordination

Support Connection

Specialist Support Coordination

Support Coordination

This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.

Support Coordination

Specialist Support Coordination

Support Coordination

This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Specialist Support Coordination

Specialist Support Coordination

Specialist Support Coordination

This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

What We Provide

Level 2 - Support Coordination

  1. Build Support Network - Our Support Coordinator will link you with relevant supports included in you plan such as a Mental Health Peer Worker, Therapy Supports, and Allied Health Professionals. We will ensure that your support network will include formal and mainstream supports that can assist in building your capacity. As an unregistered provider, we will help strengthen your informal supports by allowing them to be part of your journey.
  2. Fund Utilisation - Understanding your funding and utilizing it is an important part of a Support Coordinator's role. As part of our responsibility to you, we will try and find the best possible supports in a cost effective way while enabling you to exercise your choice and control. Through this, we can ensure that you have available funding for the duration of your NDIS plan. We will keep you up to date with service agreements and support schedules.
  3. Plan and Review - Plan reviews are stressful times for participants and families. Anxiety can spark unwanted emotions. As your Support Coordinator, we will assist you to prepare for your plan reviews ensuring that all reports and/or evidence are available. We will aim to identify barriers in achieving your NDIS goals and implementing your plan.

Psychosocial Recovery Coaching

Empowering Lives Through Psychosocial Recovery Coaching

Psychosocial Recovery Coaches are dedicated professionals funded under the NDIS Capacity Building category, specifically Support Coordination. While both Recovery Coaches and Support Coordinators share the goal of ensuring participants make the most of their funding by connecting them with service providers and community supports, Recovery Coaches go beyond coordination. They empower participants to actively work on their mental health and overall well-being, utilizing both NDIS-funded supports and mainstream services.

A Psychosocial Recovery Coach, as defined by the NDIS, is specifically tailored to support individuals with psychosocial disabilities. These professionals collaborate closely with participants, their families, carers, and other services to provide holistic support. Recovery Coaches help participants navigate life’s complexities while fostering independence, self-confidence, and personal growth. They empower individuals to take control of their lives, develop resilience, and overcome day-to-day challenges.

Participants can choose a Recovery Coach with lived experience of psychosocial disability or one with extensive professional knowledge and expertise in mental health. This personalized approach ensures that participants receive support that aligns with their unique needs and circumstances.

It’s important to note that NDIS plans cannot fund both Support Coordination and Recovery Coaching simultaneously. Participants have the choice to select the support that best meets their goals and aspirations. With a focus on mental health, collaboration, and empowerment, Recovery Coaches are transforming lives, helping individuals regain control and achieve meaningful progress toward their recovery and life goals.
